  • Be 18 years of age or older.
  • Provide an Employment Application or Resume.
  • Child Abuse Clearance form from Compass
  • Criminal Record Clearance from PATCH
  • FBI Fingerprint Clearance form: from www.identogo.com Use service code: 1KG8TG.
  • A Completed Form I-9, Employment Eligibility Verification
  • Copies of identification documents that satisfy the I-9 requirements. (see Form I-9 info. above).
  • Have a valid State Driver’s License.
  • Have a current State motor vehicle registration.
  • Provide proof of automobile insurance.
